To apply offline, you will have to go to your nearest Janpad Panchayat or Municipal Corporation office and get the application form from there and fill in all your required information and attach all your required documents.
After this you will have to submit the application form in the Janpad Panchayat or Municipal Corporation office, after which your application form will be checked by the concerned officials and if you are found eligible, you will be given the benefit of the scheme.
